Terms & Conditions

I give permission for my child to attend for training, grading and competitive sessions. 

I will inform the Instructors/ coaches of any important changes to my child’s health, medication or needs and also of any changes to our address or phone numbers given.

In the event of illness, having parental responsibility for the above named child, I give permission for medical treatment to be administered where considered necessary by a nominated first aider, or by suitably qualified medical practitioners. If I cannot be contacted and my child should require emergency hospital treatment, I authorise a qualified medical practitioner to provide emergency treatment or medication.

* Parental consent. A mother always has parental responsibility. A father has parental responsibility if:

• He was married to the mother when the child was born
• He later married the mother
• He was registered as the father on the birth certificate of the child born after 1st December 2003
• He has made a ‘parental responsibility agreement’ with the mother, or a court has ordered that he has parental responsibility 

I give permission for club promotional material to be created and used by Trodai Academy to promote and advertise the club
